Ontario Health Teams collaboration welcomes HealthPathways in Canada
Three Ontario Health Teams; Burlington Ontario Health Team, Greater Hamilton Health Network, and Middlesex London Ontario Health Team are thrilled to announce, in collaboration with Streamliners, the launch of HealthPathways in Canada. The agreement marks a milestone for integrated health systems across three regions by introducing a proven model that enhances health outcomes through a collaborative approach with our local healthcare providers.

Ontario Health Teams are providing a new way of organizing and delivering care that is more connected to patients, families and care partners in their local communities. Healthcare providers work as one coordinated team – no matter where they provide care, to strengthen local services, make it easier for patients to navigate the system, and create seamless transitions between providers.

Integrated health for better outcomes
HealthPathways is a trusted, clinician-designed tool that supports healthcare professionals by providing locally relevant, evidence-based guidelines and resources. The introduction of HealthPathways in Canada enables clinicians to deliver more efficient and effective care, improving patient outcomes across the healthcare system. “We’re thrilled to welcome Greater Hamilton, Burlington, and London Middlesex, three Ontario Health Teams into our global community. Local teams, global expertise
As a global team headquartered in Christchurch, New Zealand, Streamliners is looking forward to leveraging its worldwide team and expertise. The Canadian rollout will be led by the UK chapter of Streamliners. Dr David Hambleton, Managing Director of Streamliners UK says:

The on-the-ground presence of Streamliners UK will ensure that HealthPathways remains tailored to the specific needs and challenges of Canadian clinicians and patients, while also

tapping into Streamliners’ international expertise. This expansion broadens the global HealthPathways Community and brings Canadian healthcare perspectives into an international dialogue, benefiting all members of the HealthPathways Community.
